The CAPS Provider Inquiry System (CPIS) is provided to you by the Montana Department of Public Health and Human Services.

You must complete an access request form in order to receive a password which will enable you to perform CAPS warrant and payment queries or submit invoices to the CAPS system. Entry into the system requires a valid provider/facility number and password. Each provider/facility number requires a unique password. Therefore, a separate request form must be completed for each facility.
